    Finding a Health Plan

    Use this section to understand more about choosing a health plan, and see a list of health plans accepted by Memorial Medical Center.

    We strive to keep this information as complete and current as possible. It is a good idea to contact your health plan Customer Service Department regarding the specifics of your plan type and coverage.

    The Sutter Health and Blue Shield agreement expired on December 31, 2014. We have now entered into a six-month transition period of our agreement. Please check with Blue Shield directly by calling the customer service number on your health plan identification card or 1-800-200-3242 to ask how the transition agreement applies to you and your family.

    In addition, the Care1st Medicare Advantage contract expired on December 31, 2014. Please contact Care1st directly for information regarding transition of care.

    It is important to note that not all of the Sutter Health network of providers participate in all of the health plan's products or networks. Please check with your health plan if you have questions about benefits, a specific product, or a provider network.
