Memorial Medical Center offers additional support to survivors through the Patient Navigation Program. Oncology Nurse Navigators and Cancer Peer Navigators are available to assist survivors by providing education, resources and emotional support at any point of survivorship.
CANCER PEER NAVIGATORS:
For some cancer survivors, it may not be enough to have the support of family, friends, and the professional healthcare team. Sometimes a survivor needs to talk to someone who has experienced the emotional and physical journey that comes with a cancer diagnosis and survivorship. Memorial Medical Center’s Cancer Peer Navigator program may be able to provide the additional support needed. Cancer Peer Navigators are survivors who have been trained to provide one-on-one support and help empower others through diagnosis, treatment, and survivorship.
Cancer Peer Navigators may be able to provide support by:

- Being alongside you on your cancer journey

ONCOLOGY NURSE NAVIGATORS:
An oncology nurse navigator is an oncology certified nurse (OCN®) who can help patients and families navigate through cancer treatment and survivorship. The nurse navigator’s goal is to help improve the cancer experience and is available through every stage of life.
We currently offer nurse navigator services to adult cancer survivors in our community, regardless of where he or she receives care, at any point of the cancer journey. The nurse navigator can help survivors understand their diagnosis, treatment options, discuss symptoms of treatment and help with community and online resources. The nurse navigator can also help patients make informed decisions regarding care, offer emotional support, and stay on track with treatment and follow-up.
