About VNA Continuous Care

Following an illness or injury, many people require additional assistance to recuperate or maintain their independence. VNA Continuous Care offers a variety of services which include supplemental care at home as well as in the hospital, providing care for older and disabled persons, working families when a loved one is sick, and much more. An individualized plan of care is developed for each client.

Staff

All VNA employees are bonded, screened, drug tested and possess current CPR certificates and health screenings.

Certified nurses aides are required to have a minimum of six months of experience.

An on-call supervisor is available for emergency staffing issues when the office is closed, to ensure that our clients receive their scheduled care as soon as possible.

Rates*

Hospital Sitter Service
Personal care, companionship, comfort provided in the hospital when family can’t be there.
Nurses Aides
Bathing, personal grooming, dressing, exercising, light housekeeping, laundry, shopping, medication reminders, meal preparation.
Sleep-Over
Bedtime and wake-up assistance, overnight stays (ask for details).
Bathing Care
Tub, shower or bed baths, dressing assistance, shaving, shampooing and linen change.
Licensed Nurses
100% refund if service not used. One week deposit required at time of service.
HealthWatch
Emergency response system.

Health Watch Emergency Response System

This affordable service enables you to access help when you need it most, 24 hours a day. This program is available for Stanislaus County residents of all ages, from latchkey kids to seniors.
Special features include:

  • Compact personal transmitter that may be worn around the neck, on the wrist or with a belt clip
  • Emergency assistance at the press of a button
  • Fall detector
  • Medication and personal reminder
  • Automatic daily self-test to ensure proper system function
  • Wellness check-in
  • Convenience calling to answer incoming phone calls

MD.2 Personal Medication System

Now there's an easier way to manage all those pills with a personal medication dispenser. The medication system is programmed to dispense your medication and supplements at scheduled times.
Special features include:

  • Medication dispensing up to six times per day for 30 days
  • Medication reminders announced by a loud, clear voice and tone, text message and flashing red light
  • Phone monitoring and support of system if medication is not dispensed after multiple reminders
  • Reminder if medications run low

Services to outlying areas

An outlying area is defined as one that is 15 miles or more from the VNA Continuous Care office. These areas include, but are not limited to Denair, Hilmar, Hughson, Manteca, Newman, Oakdale, Patterson and Turlock. VNA services provided in these areas are reflected at the highest billing rates.
